Supporting Transition

At Mayfield We recognise the importance of all transitions across your child's time with us in school. This can range from transition from home to school in the morning to planning for leaving college at 19.We support our pupils and families at each step of the way with the key transitions. As students move towards adulthood this takes on more importance to ensure once our students leave they move onto the most relevant destination.

Transition Planning from Year 9 onwards.

We recognise that transition periods can present a challenge for our students and families, especially in relation to those big decisions about what to do or where to go next. Rest assured we make it our priority to keep you informed about all your options at each stage of your child's journey with us. This is why we have a dedicated transition worker- Julie Hurst who will work alongside families and consult with outside agencies to support effective decision making. 

Families will be invited to discuss transition plans during EHCP reviews from Year 9. See the transition flow chart below.
